11 years ago a 28 year old man defended a woman. In the ensuing bar fight he was injured so badly that now he has control of only his eyes and a tiny bit of ability to smile.

Where do I go first to get him a computer interface of eye-tracking and click-by-smile mouse?

I have a friends who works for the Nuffield Orthopaedic Centre in the Oxford (in the UK).

You might like to contact their assistive technology dept. for advice as they deal with things like this all the time (custom builds based on injury), and nearly always get something that works for the client.
